Vertigo is a false sensation of spinning, often caused by diseases of the inner ear. It can affect people of all ages and gender. Simple movements like turning in bed or looking up can trigger it. It may interfere with daily activities and impair your overall quality of life.

Though it can feel alarming, most causes are treatable when diagnosed accurately. Many patients live with it longer than necessary due to delayed diagnosis.

Early evaluation is the key. Timely care not only relieves the symptoms, but also helps patients regain confidence and stability.

At CHESS Surgical Centre, we evaluate them clinically during the first visit with detailed history and complete clinical examination. Initial examination helps us narrow our diagnosis and provide the patient with only the necessary treatment. All patients with giddiness, dizziness, or swaying do not have true vertigo, and they should not be treated with anti-vertiginous drugs. A simple postural fall of blood pressure can also cause giddiness. This is where our centre stands unique.

Cranio-Oculography (COG)

Videonystagmography (VNG), now recently improvised as Cranio-oculography (COG), is an advanced diagnostic test to evaluate the cause of vertigo. It records and analyses the eye movements, which are closely linked to the inner ear. When the vestibular (balance) system is affected, it sends abnormal signals to the brain, which show up as specific patterns of eye movements. It helps the clinician identify whether the patient's problem is arising from the inner ear or any other part of the rest of the balance system (cerebellum and its connections).

This is a non-invasive test, which provides additional objective information that a routine examination cannot give. COG allows for targeted treatment, helps patients recover faster, and avoids unnecessary medications.

Instructions Before Your COG Test

• Avoid or stop any vertigo medicines or sedatives 48 hours prior to testing.
• Avoid alcohol.
• Take a light meal 3 hours prior to testing.
• Do not apply eye make-up (eye-liner or mascara).
• If vomiting is one of your major symptoms, please inform us — we will keep sickness bags ready.
• Avoid gel or liquid bindis on your forehead.
• Always come with your attender.
• It is normal to feel slight uneasiness after testing, hence kindly travel back home in a four-wheeler.
• Testing will take 45 minutes to one hour for each patient, so come at the particular slot given to you and make sure you have adequate time in hand. Kindly do not hurry us.

Rhinoplasty is a surgical procedure to reshape the appearance of the nose, for cosmetic or functional correction. It can address concerns like hump nose, crooked nose, broad nasal bridge, drooping or bulbous tip, and nasal obstruction due to nasal valve deformities. The procedure is tailored to each patient depending upon their complaints, clinical findings, and expectations. In addition to cosmetic enhancement, rhinoplasty often drastically improves the airway, helping you breathe comfortably.

Surgery can be done under local anaesthesia with sedation, or under general anaesthesia.

The Two Main Approaches

1. Closed Rhinoplasty: All incisions are made inside the nose, with no visible scars.
2. Open Rhinoplasty: A small incision is made across the columella (the strip between the nostrils) to allow better visibility and precision.

During the surgery, the surgeon carefully reshapes the nasal bones and cartilage by removing or reducing them. He may refine the nasal tip, straighten the septum, or add a cartilage graft for structural support. The graft may be harvested from the nose, or sometimes from your pinna (external ear) or rib cartilage. The surgical time is usually between 3 to 4 hours.

Recovery

Following the procedure, a nasal splint is kept for support. Mild swelling and bruising around the eyes are normal and will settle in a few days with the application of an ice pack. Patients can return to routine activities within a week, although complete healing may take up to a few months.

When performed with a balance of aesthetics and function, rhinoplasty helps patients look and breathe better.

Most important: Have realistic expectations. Rhinoplasty will not give you a perfect nose, but a natural-looking nose that suits your face.

Endoscopic CSF Leak Repair

A CSF leak occurs when there is an abnormal opening between the lining of the brain and the nose, causing watery discharge from one nostril. Left untreated, it carries the risk of meningitis. Endoscopic repair allows us to precisely localise and seal the defect through the nose, with high success rates and minimal discomfort.

Four-Hand Technique in Skull Base Surgery

Endoscopic skull base procedures at CHESS are performed using the four-hand technique — two surgeons working simultaneously through the nostrils. One surgeon stabilises the endoscope and manages suction while the other operates with both hands, giving steady visualisation, finer dissection, and safer handling of critical structures.
